In consideration of the issuance of a Calling Card(s) to the customer, the customer agrees to be bound to the following terms and conditions applicable to use of this Calling Card(s):

This Calling Card is not transferable. The customer may authorize others to use the Calling Card, but the customer remains responsible for the charges incurred by these persons.

This Calling Card is considered void and will be canceled if the telephone service or account which the charges are billed has been discontinued or changed to a responsible party other than the customer.

If your telephone number is changed or service discontinued, destroy all associated Calling Cards and notify all authorized users to guard against unauthorized charges by others. The customer remains responsible for all charges incurred by authorized users, unless all Calling Cards are returned or destroyed by the customer.

• If the Calling Card was received from an employer and a change in employment has occurred, the card should no longer be used and should be promptly returned to the employer.
• Calling Card calls are generally more expensive than direct-dialed calls. Charges for Calling Card calls and use of features are subject to change.
• You may be liable for the unauthorized use of your Calling Card. However, you will not be liable for unauthorized use that occurs after you notify Cbeyond, orally or in writing, of any loss, theft or possible unauthorized use.
• Cbeyond reserves the right to change or cancel a Calling Card when fraud is suspected. Continued use of the Calling Card after cancellation may constitute a violation of law.
• The terms of this Agreement may be changed from time to time and use of this card after notification of those changes constitutes acceptance of the new terms.
• Use of this Calling Card(s) is an extension of credit and the customer agrees to pay for all charges incurred.
• Charges for use of this card include a per minute usage charge and may include a per call surcharge. Contact Cbeyond customer service at 1-866-424-5100 for rate information.
• Calling Card calls may be placed to and from any location in the United States and Canada. Calls may be placed from the U.S. to international locations and from certain international locations to the U.S. and to other international locations. Please note some international locations may be blocked.
